Welcome to Clear Horizon Wellness. By using this website, scheduling an appointment, or receiving our services, you agree to comply with and be bound by the following terms and conditions.

 

 Service Agreement Nature of Services: Clear Horizon Wellness provides professional, therapeutic massage therapy services. Our services are intended for relaxation, stress reduction, and pain relief from muscle tension.

 

Non-Medical Services: Our services are not a substitute for medical examination, diagnosis, or treatment. Our therapists do not diagnose illnesses or diseases, prescribe medications, or perform spinal manipulations. You are advised to consult with a primary care provider for any mental or physical conditions.

 

In-Home Service Conditions: You agree to provide a clean, safe, and professional environment for the massage session. We reserve the right to refuse or terminate a session if the location is deemed unsafe or unsanitary.

 Appointments, Cancellations, and No-Shows Booking: All appointments are subject to availability and must be scheduled in advance through our website or by contacting us directly.

 

Cancellation Policy: We require at least 24 hours' notice for all cancellations or rescheduling requests. Less than 24 hours' notice: If you cancel with less than 24 hours' notice, you will be charged a cancellation fee of [e.g., 50% of the session fee or a flat fee].

No-Shows: If you fail to show up for your scheduled appointment without any notification, you will be charged 50% of the full price of the scheduled service.

 

Late Arrivals: We regret that late arrivals will not receive an extension of their scheduled appointment time. The full service fee will still apply. If you anticipate being late, please contact us immediately.

 

Termination by Therapist: A session may be ended at any time if the therapist feels unsafe or if client conduct is inappropriate. In such cases, the full session fee will be charged, and no refund will be issued.

 

 Client Health Information and Consent Disclosure of Health Conditions: By receiving a massage, you affirm that you have disclosed all known medical conditions, injuries, and medications to your therapist. You agree to keep your therapist updated on any changes to your health profile for current and future sessions.

 

Informed Consent: By booking an appointment, you acknowledge that you have been given the opportunity to ask questions regarding the massage and that any questions have been answered to your satisfaction. You consent to receive massage therapy and agree to inform the therapist of any pain or discomfort experienced during the session.

 

Confidentiality: All client information is kept strictly confidential in accordance with our Privacy Policy. For more information, please refer to the policy posted on our website.

 

Payments Payment Methods: We accept payment via cash, check and Venmo.

 

Payment for In-Home Services: Payment is due at the time of service. For convenience, a deposit of %50 through Venmo is required to hold your booking. You will be charged according to our cancellation policy if you violate the terms.

 

Professional Boundaries and Conduct Respectful Environment: We maintain a strictly professional, therapeutic environment. Any illicit or sexually suggestive remarks, advances, or behaviors made by a client will result in immediate termination of the session and a report to the proper authorities. The full session fee will still be charged.

 

Therapist Boundaries: You understand that the massage therapist reserves the right to refuse to perform massage on anyone they deem to have a condition for which massage is contraindicated.

 Limitation of Liability By using our services, you agree that you understand the risks and acknowledge that there shall be no liability on the practitioner's part due to your failure to disclose pertinent information or for issues that arise during an in-home session, provided the therapist was acting professionally.

 

Minnesota Complementary and Alternative Health Care Client Bill of Rights As an licensed complementary and alternative health care practitioner in Minnesota, we must comply with and post the Client Bill of Rights. We will provide you with a copy of this document.

Governing Law and Jurisdiction This agreement and your use of our website and services are governed by and construed in accordance with the laws of the State of Minnesota. Any disputes arising from these terms will be resolved in the courts of Minnesota.

 

Modifications to Terms We reserve the right to update these terms and conditions at any time. The most current version will be posted on our website. Your continued use of our services indicates your acceptance of the revised terms.10.
